Perspectives hormonales sur les différences genrées et sexuées : un essai sur les profils atypiques des femmes schizophrènes

Abstract

Les différences genrées et sexuées (G/S) contribuent à la forte hétérogénéité clinique des troubles du spectre psychotique, pourtant la question de l’influence endocrinienne reste ouverte. Quatre hormones (testostérone, œstradiol, progestérone, prolactine) ont été étudiées afin d'identifier d'éventuels profils atypiques et sous-groupes cliniques. Le travail de mémoire dont cet essai présente une adaptation, comparait des patient.e.s SCZ et contrôles sur plusieurs variables hormonales au moyen de l’examen des proportions d’hyperprolactinémie présentes dans chaque groupe et de l’hétérogénéité clinique grâce à une analyse par clustering hiérarchique. Conformément à la littérature antérieure, l'hyperprolactinémie était plus fréquente chez les hommes SCZ et tendanciellement plus élevée chez les femmes, malgré une association ténue avec les antipsychotiques. Enfin, l'analyse par clusters a identifié un sous-groupe de femmes présentant des profils cliniques saillants accompagnés de taux d'œstradiol plus faibles, pas mieux expliqués par un effet iatrogène. Pris ensemble, ces résultats soutiennent une vision intégrative et sensible au G/S de la SCZ, éclairée par des analyses endocriniennes, qui souligne l’importance d’ouvrir de nouvelles voies pour les soins personnalisés et des stratégies de traitement sur mesure.
