A PSYCHOSOMATIC PERSPECTIVE ON ENDOMETRIOSIS – A MINI REVIEW

Résumé

Objectif: L’endométriose est une affection médicale extrêmement invalidante et à long terme, caractérisée par la présence de tissu semblable à l’endomètre à l’extérieur de l’utérus, se manifestant généralement par des crampes, des douleurs menstruelles et ovulatoires, des douleurs abdominales inférieures et/ou pelviennes, une fatigue chronique, une dyspareunie et une infertilité. L’endométriose est souvent comorbide avec une détresse psychologique qui aggrave la qualité de vie des femmes touchées. Méthodes: L’objectif principal était d’examiner les preuves actuelles sur la prise en charge de l’endométriose d’un point de vue psychosomatique. La littérature a été synthétisée et résumée dans un format narratif. La recherche documentaire a été effectuée à l’aide de PubMed, à partir de rapports, d’articles de synthèse, de méta-analyses, de synthèses et de lignes directrices publiées en anglais au cours des 15 dernières années. Résultats : Cinq sujets principaux sont abordés et discutés : la pathogenèse de l’endométriose, l’endométriose et les caractéristiques psychologiques, l’endométriose et la comorbidité psychiatrique, le stress en début de vie comme facteur prédisposant à la sensibilisation à la douleur ; et les implications pour le traitement. Conclusions : La littérature disponible souligne clairement qu’il est important de considérer et de traiter la comorbidité de l’endométriose avec des facteurs psychologiques qui affectent la gestion de la douleur.
