What profile for involuntary hospitalizations in psychiatry?

Abstract

Psychiatry has a special place among the medical specialties concerning hospitalizations against the will of the patient.  Until 2012 it was deprivation of liberty for the purpose of assistance. From 1.01.2013 a new Civil Code was adopted in Switzerland. Articles 426 to 429 introduce placement for the purpose of assistance (PPA), a protective measure for the person. Our retrospective study of 2014-2016 has as objectives (1) to bring knowledge of certain parameters explaining non-voluntary hospitalizations; (2) to answer the question: Can we consider this type of hospitalization as a clinical experience? The main results demonstrate that the significant factors at a p-value level ≤ 0.05 that increase the likelihood of having an PPA are: (a) living in a foster home ; (b) be diagnosed as having psychotic disorders comorbid with substance abuse; being male and living alone, however the age variable (p = 0.03) decreases the likelihood of having a PPA (meaning that the older you are, the more likely you are to have a PPA is low).  The coercion exercised, the legal status and the constraint felt by the patient and the psychiatrist are three dimensions to be integrated into our reflection. Forced hospitalizations find an exceptional place in the medical field of psychiatry, yet they are not inevitable. The ethical, legal and clinical implications of involuntary hospitalizations shed particular light on this type of psychiatric care.
